I acknowledge that I am expected to attend all classes of the training course program I hereby now sign up to complete. I understand that I am not entitled to makeup sessions, and that these special sessions are provided only at the discretion of the trainer.
I acknowledge that I am expected to attend all private appointments during their scheduled times.
I acknowledge that I am responsible for completing all homework assigned by the trainer during the service period including but not limited to reading electronic materials in full and engaging in regular practice sessions with my pet.
I commit to continue the behavioral structure outlined by the training program after service period ends in order to maintain effectiveness of training. I recognize that Canine PhD is not responsible for lapses of learned behavior during or after the service period.
I acknowledge that the trainer will be in touch with me by email throughout the program. I understand that additional services, including but not limited to email, telephone, and video conferencing support, are available for purchase from Canine PhD, should I need assistance beyond the scope of the program.
I agree to discuss any concerns or complaints with the trainer or business manager directly. I agree to make any formal complaints in person so that Canine PhD can provide the best level of service.
Separation Anxiety training: I am making a 4 week commitment to suspend absences so that my dog does not panic during alone time.
I agree that I am making a commitment to complete daily Missions.
I understand that if I do not complete my Missions and provide feedback to my CSAT, I will impede the training process and prevent behavior modification.
I grant to Canine PhD Academy, its representatives and employees the right to take photographs and/or video of me, my dog(s), friends, and family members (including minor children). I authorize Canine PhD Academy, its assigns and transferees to copyright, use and publish the same in print and/or electronically. I agree that such photographs, video recordings, and audio recordings of me with or without my name may be used for any lawful purpose, including for example such purposes as publicity, illustration, instructional, educational, advertising, and Web content.
Payment for the initial consultation is due at the appointment and is non-refundable.
Once a training program has been determined and agreed upon, I understand that I have committed to the full length of the training program and that I am responsible for payment in full regardless of whether I choose to complete the training program.
All training packages expire after 6 months of the purchase date.
There is an extra fee for travel more than 10 miles. This fee will be factored into your lesson rate.
Because our goal is for your dog’s behavior to change and because we want your dog to achieve the best outcome from training possible, we highly discourage cancellations. The importance of your dog’s training sessions cannot be stressed enough. Please schedule them carefully. We appreciate your cooperation in maintaining consistency in your dog’s training.
CONSULTATIONS, GROUP CLASSES, INDIVIDUAL LESSONS, AND TRAINING PACKAGES ARE NON-REFUNDABLE AND NON-TRANSFERABLE.
